Debts That Can Block a Visa: NHS, Tax and Child Support

A £600 hospital bill left unpaid can cost you a UK visa. A large credit card balance, on its own, usually will not. Immigration rules care far more about money you owe a government than money you owe a bank.

If a debt question on a visa form has made your stomach drop, this guide shows which debts matter, where, and how to clear them.

The short answer: As of October 2026, most private debts, such as credit cards or a personal loan, do not block a visa by themselves. But some debts to governments can. In the UK, an unpaid NHS hospital bill of £500 or more, unpaid for 2 months, can lead to a visa refusal (GOV.UK Immigration Rules, Part Suitability, October 2026). Australia can refuse a visa if you owe money to the Australian Government and have no repayment plan (Migration Regulations 1994, Schedule 4, 2026). In the US, owing more than $2,500 in child support stops you getting a passport (US Department of State, May 2026). In almost every case, the fix is the same: repay, set up a payment plan, or declare it honestly.

Which debts count, country by country

The pattern is clear: the rules target money owed to the state, not to private lenders.

The UK: NHS debt and the £500 line

If you are an overseas visitor and no exemption applies, NHS hospitals in England must charge you for most hospital care. The charge is usually 150% of the cost of treatment (DHSC, May 2026).

How a bill becomes an immigration problem

You get 2 months from the date on your invoice to pay. If you do not, the debt may be added to your immigration record (Home Office, 2023). The Home Office then says you can be refused a visa or stopped at the UK border.

Which debts count depends on when you were treated:

  • Most nationalities: £500 or more for treatment since 6 April 2016, or £1,000 or more for treatment from 1 November 2011.
  • EU, Norway, Iceland or Liechtenstein citizens: £500 or more for treatment since 1 July 2021.

Many readers look for this in "Part 9" of the Immigration Rules. As of October 2026, the rule sits in Part Suitability, paragraph SUI 16.1. It says an application "may be refused". So refusal is not automatic. NHS guidance says the Home Office is "not compelled" to refuse. It may find a refusal unfair for compassionate or human rights reasons (DHSC, May 2026).

The good news

The debt comes off your immigration record once you either pay in full or agree to pay in instalments (Home Office, 2023). If you then miss payments, it can go back on. You pay the NHS trust that treated you, not the Home Office.

One thing to remember: urgent or immediately necessary care "must never be withheld or delayed", even if you cannot pay (DHSC, May 2026). Never avoid a hospital out of fear of a bill. To compare care costs in other countries, see our guide to out-of-pocket healthcare costs by country.

Australia: PIC 4004 and debts to the Commonwealth

Many Australian visas, including visitor, student and skilled visas, include a test called public interest criterion 4004. Its full text is one sentence: "The applicant does not have outstanding debts to the Commonwealth unless the Minister is satisfied that appropriate arrangements have been made for payment" (Migration Regulations 1994, Schedule 4, 2026).

You pass if you owe nothing, or if you still owe money but have a payment arrangement the department accepts. So the honest path is open even if you cannot pay everything today. Contact the agency you owe, agree a plan in writing, and keep the proof.

Leaving Australia with a debt

If you live in Australia now, this matters. The Australian Taxation Office can issue a departure prohibition order if you have an unpaid tax debt. You must not leave Australia while it is in force. Border staff are told, and leaving in breach of the order is an offence (ATO, June 2026).

Services Australia can do the same for a child support debt, and it does not need a court order (Services Australia, 2026). The order lifts when you pay in full or agree an acceptable payment arrangement. Planning a move abroad? Settle or arrange these debts first.

The US: child support, tax debt and public charge

The US rules affect two groups: American citizens who want to travel or move abroad, and people applying to live in the US.

For US citizens: your passport

If you owe more than $2,500 in child support, you cannot get a new passport. The State Department may also revoke a valid one. In May 2026, it said it was working with Health and Human Services "on an unprecedented scale" to do this. Eligibility comes back only after you pay the state child support agency and are no longer listed as delinquent (US Department of State, May 2026).

Federal tax works in a similar way. The IRS can certify a "seriously delinquent tax debt" to the State Department. For 2026, that means more than $66,000, including penalties and interest. Debts paid on time under an installment agreement or an accepted offer in compromise are not certified. If you apply with a certified debt, your application is held open for 90 days so you can pay or arrange payment (IRS, 2026).

For people moving to the US: public charge

A person applying for a green card from inside the US can be refused if they are "likely at any time to become a public charge". A new rule cancelled the 2022 public charge rule. It applies from 18 September 2026 (USCIS, August 2026).

Officers must look at five factors: age, health, family status, "assets, resources, and financial status", and education and skills. They can now consider any means-tested benefit received on or after 18 September 2026. For earlier benefits, they only count cash assistance and long-term institutional care paid by the government.

Most family applicants also need a sponsor who signs Form I-864. This is a legal contract. The sponsor must show they can keep you at 125% or more of the Federal Poverty Guidelines. Officers may also look at the sponsor's "financial history, including a history of bankruptcy or failure to meet his or her financial responsibilities" (USCIS Policy Manual, 2026).

Canada: sponsorship defaults and support payments

Canada's debt rules mostly affect sponsors, the people who bring family to Canada.

Outside Quebec, when you sponsor a spouse or partner, you promise to support them for 3 years from the day they become a permanent resident. For a dependent child under 22, it is 10 years or until the child turns 25, whichever comes first. Quebec has its own rules. If the person you sponsored gets social assistance during that time, you are in default (IRCC Guide 5289, 2026).

According to the same guide, you cannot sponsor while:

  • you have not repaid social assistance from a past sponsorship, in full or "to the satisfaction of" the authority that paid it;
  • you are in default on an immigration loan or a performance bond;
  • you have not paid court-ordered alimony or child support;
  • you have a bankruptcy that is not discharged;
  • you receive social assistance for reasons other than a disability (IRCC, 2026).

For the person applying, Canadian law has a separate test. You can be inadmissible "for financial reasons" if you cannot or will not support yourself and your dependants, without relying on social assistance (Immigration and Refugee Protection Act, s. 39, 2026). Moving after a separation? See our guide on relocating abroad with children.

Private debts, court judgments and the Schengen area

What about a credit card, a car loan or a court judgment from a private lender? In the rules we checked for the UK, Australia, the US and Canada, these are not a refusal ground on their own. But they still matter. Repayments reduce the money you can show for proof of funds (try our proof of funds calculator). A sponsor's bankruptcy can matter in Canada and the US. And if a form asks about debts or court cases, you must answer truthfully.

The Schengen area has no general debt check. But at the border you must show "sufficient means of subsistence" for your stay and your return (Schengen Borders Code, Article 6, EUR-Lex).

A hidden debt costs more than an unpaid one

A debt can usually be fixed. A lie is much harder to undo.

In the UK, an application must be refused if the applicant used deception, including "not disclosing relevant facts". Using deception in an application can lead to a 10-year refusal period for future applications (Part Suitability, SUI 9.1 and SUI 12.1, 2026). In Canada, misrepresentation, including "withholding material facts", can make you inadmissible for 5 years (IRPA, s. 40, 2026).

Compare the two. A £600 NHS bill on a payment plan can come off your record. A hidden £600 bill, found later, can cost you a decade. Already refused? Read about appealing, reapplying or changing country, and about overstays and re-entry bans.

Seven steps to a clean record

  1. Find out exactly what you owe. Ask each body for a written statement: the NHS trust, tax office, child support agency or welfare office.
  2. Check if it is a "qualifying" debt. For the UK, check the amount, invoice date and 2-month window. For the US, check the $2,500 and $66,000 lines.
  3. Pay in full if you can. This is the fastest way to clear a record or a passport block.
  4. If you cannot, ask for a payment plan. Get it in writing.
  5. Keep every receipt and keep up the payments. A missed instalment can put a UK NHS debt back on your record.
  6. Answer every form question truthfully. Declare the debt and attach proof of the plan.
  7. Allow time. The IRS says it reverses a certification within 30 days of the debt being resolved (IRS, 2026). Build this into your timeline. Our visa costs tool helps you budget for the whole move.

"It won't follow me abroad"

Many debts come from illness, a broken relationship or a lost job. A hospital bill after an emergency is not a moral failure. But the dream of a new country can carry a risky idea: once I leave, this stays behind.

For government debts, it usually does not. Australia may stop you leaving. The US may take your passport. The UK keeps the bill on your immigration record. A real fresh start means leaving with every debt paid or on a plan, and nothing hidden.

Six mistakes that turn a bill into a refusal

  • Ignoring the letters. Unopened mail removes your chance to agree a plan early.
  • Hiding the debt on a form. This turns a fixable problem into a deception problem.
  • Thinking a payment plan is not enough. In the UK and Australia, an agreed plan is often what the rules ask for.
  • Leaving to escape creditors. Under a departure order in Australia, this is an offence. It rarely solves anything.
  • Forgetting old bills. UK NHS debts of £1,000 or more invoiced from November 2011 can still count.
  • Sponsoring while in default. In Canada, your application can fail before anyone looks at your partner.

When a debt needs a professional

Many people can fix a simple debt alone. Get regulated help if:

  • you were already refused for a debt, deception or misrepresentation;
  • a debt is disputed, or you think it was added to your record by mistake;
  • you face a departure prohibition order or passport revocation;
  • the debt involves child support across two countries;
  • your US case involves public charge questions or past benefit use;
  • you are in default on a Canadian sponsorship and want to sponsor again.

Use a regulated professional: a solicitor or Immigration Advice Authority adviser in the UK, a registered migration agent or lawyer in Australia, a lawyer or licensed consultant in Canada. Our guide on whether you need a migration agent explains the options.

Frequently asked questions

Can an unpaid NHS bill stop me getting a UK visa?

Yes. If you owe £500 or more for 2 months, the NHS may report it. The Home Office may then refuse your visa or stop you at the border, but refusal is not automatic. Paying in full or agreeing instalments removes the debt from your immigration record.

Does credit card debt affect a visa application?

In the UK, Australian, US and Canadian rules we checked, ordinary private debt is not a refusal ground by itself. It can still reduce the funds you are able to show. And if a form asks about debts or court cases, you must answer honestly.

What is PIC 4004 in Australia?

It is a public interest test in Schedule 4 of the Migration Regulations 1994. It says you must not have outstanding debts to the Commonwealth, unless the Minister is satisfied you have made appropriate arrangements to pay. A written payment plan can let you meet it.

Can I lose my US passport for unpaid child support?

Yes. If you owe more than $2,500, you cannot get a new passport, and a valid one may be revoked. You must pay the state child support agency and be cleared in federal records before you can get a passport again.

Can I sponsor my spouse to Canada if I owe child support?

Not while you are behind on court-ordered support payments. IRCC says you cannot sponsor until you resolve the family support matter. The same applies to unpaid social assistance from an earlier sponsorship and to a defaulted immigration loan.

Will a debt follow me if I move abroad?

Government debts often do. The UK keeps NHS debts on your immigration record. Australia can stop you leaving, and the US can block your passport. The safest plan is to repay or arrange payment before you go.
