Registration, English Test, Employer, Visa: The Order That Saves Overseas Nurses a Year

For an overseas nurse moving to the UK or Australia, the working order is English test first, then qualification and registration verification, then the regulator's exam or assessment, then the employer and job offer, then the police clearance and medical, and the visa application last. The English test comes first because it is the cheapest way to find out whether either route is open to you, and because both the Nursing and Midwifery Council (NMC) in the UK and the Australian Health Practitioner Regulation Agency (Ahpra) require an English result before they will progress a registration file. The police clearance and the medical come last because they are the shortest-lived documents in the whole set, and issuing them early is how nurses end up paying for them twice.

The trap is that these steps have different lifespans. An English test result is typically accepted by health regulators for a limited period counted from the test date, commonly around two years, and the immigration department may apply a different window to the same certificate. Verification of your qualification and home registration can also be treated as valid for a limited period. A police certificate and a visa medical are usually valid for a matter of months. Sit the English test, spend nine months finding an employer, then wait five months for a visa decision, and the certificate that opened the door can lapse before the door closes behind you.

This is general information for planning. Every validity window, fee and processing time named below is a planning range as checked in August 2026, and each must be reconfirmed on the issuing authority's own page before you book, budget or apply. A registered migration agent or regulated immigration adviser should confirm the visa half of your plan.

Who this sequencing guide is for

This is written for a nurse who qualified and registered outside the UK, Australia, Ireland, New Zealand, the United States or Canada, who is currently practising, and who is planning a first move abroad. It applies most directly to nurses trained in India, the Philippines, Nigeria, Pakistan, Kenya, Ghana, Nepal, Egypt and Sri Lanka, because those qualification origins most often trigger a full assessment path with both regulators.

If you have not yet chosen between the two destinations, read the NMC and Ahpra comparison first and come back. Sequencing only helps once you have picked a target. Running both registers in parallel doubles your fees and the number of expiry dates you have to defend.

What you need before you start

Assemble the inputs everything else depends on before you spend anything:

  • Your original nursing qualification documents, transcript and certificate, held by the awarding institution in a form it can send directly to a regulator.
  • Your current home-country registration or licence, in good standing, with the council able to issue a certificate of good standing on request.
  • A verifiable record of post-registration clinical practice, with dates, employers and hours, backed by letters a hospital HR department can confirm.
  • A passport with enough validity to carry you through a visa grant and beyond.
  • A realistic figure for how many months of unpaid waiting your savings can absorb. This number decides your sequence more than anything else on the list.

The dependency chain, explained

Some steps block others and some run in parallel. The order below reflects what genuinely depends on what.

English test. Both regulators publish an English requirement, and it usually carries sub-skill minimums in listening, reading, writing and speaking. OET and IELTS Academic are the two tests most commonly named in regulator guidance, and the regulator's bar is commonly higher than the immigration department's. Both facts are published by the regulators themselves and are revised, so confirm the accepted tests and the required scores on nmc.org.uk or ahpra.gov.au before you book. Sitting this first tells you within weeks whether the route is realistic. It also starts the certificate's clock, so read the expiry section below before you book. Qualification and registration verification. The step you control least. It involves your university, your home nursing council and, for many applicants, a third-party verification service acting for the regulator. It moves at the speed of a records office in a country you may no longer live in. Start it early, alongside your English preparation.

Regulator application and assessment. The NMC route for most internationally trained nurses runs a computer-based test sat in your home country, followed by an objective structured clinical examination taken in person in the UK. Ahpra streams applicants by where they qualified and typically directs nurses outside a small comparable group into either an outcome-based assessment with an in-country practical component or a bridging program studied in Australia. Exam names, formats and the streams applicants are placed into are all set by the regulators and are revised, so read the current assessment pages on nmc.org.uk and ahpra.gov.au directly. The descriptions here are a map of the categories you will meet, and the regulator's page is the authority on today's rules.

Employer and job offer. In the UK this step is load-bearing: the clinical exam is normally sat after you arrive, and you arrive on a sponsored visa, so the employer comes before the final exam. In Australia a points-tested skilled visa needs no employer, so the job search can run after the visa or beside it. That single structural difference is why the two countries need different sequences.

Skills assessment (Australia). Points-tested Australian skilled visas require a skills assessment from the assessing authority the Department of Home Affairs names for your occupation. For registered nurses that authority has been the Australian Nursing and Midwifery Accreditation Council (ANMAC), and the department publishes the current assessing authority against each occupation, so check the list for your exact occupation code. The assessment is separate from Ahpra registration and has its own fee, evidence rules and validity period. Budget for both.

Police clearance and medical. Last. These are short-lived, and a certificate from a country you left five years ago can take months to obtain. Order them when a visa application is imminent, and check whether any country you need one from has a long queue before you assume "last" means "quickly".

Visa application. The UK Health and Care Worker visa, administered by UKVI, is sponsored by a licensed employer that assigns you a certificate of sponsorship. Australia's points-tested skilled visas, administered by the Department of Home Affairs, are lodged after an invitation that follows an expression of interest submitted through SkillSelect. Employer-sponsored Australian subclasses follow a different path. Eligibility rules, salary or income thresholds and processing times for all of these are published by UKVI and the Department of Home Affairs and change without much notice, so treat any figure you read, including here, as a planning range to reconfirm on the department's own page.

Which documents expire and which do not

This table is the heart of the problem. The left column is the thing you buy, the right column is what happens to it while you wait.

Read that table as a set of overlapping timers. Your job is to arrange the work so that no timer runs out while you wait on something else.

Two entries deserve a warning. The certificate of sponsorship and the skilled visa invitation both have short windows and arrive on someone else's schedule. When one lands, everything else in your file needs to be ready.

The UK sequence, month by month

This is a worked twelve-month plan for the UK route. It assumes a nurse starting from scratch with a current home registration and no employer. Decision points are marked.

Months 1 to 2. Prepare and sit the English test at the level the NMC publishes, checking the sub-skill minimums as well as the overall score. At the same time, ask your university and your home council what they need to respond to a verification request. Order nothing else.

Decision point A, end of month 2. If you did not clear every sub-skill, decide now whether to retake one component or change destination. Retaking is usually cheaper. Do not start paying regulator fees while an English gap is open.

Months 2 to 4. Open the NMC application, submit qualification and registration details, and let verification run. Book and sit the computer-based test. In parallel, work through the GOV.UK register of licensed sponsors and apply to NHS trusts and larger private providers that run structured international recruitment.

Months 4 to 7. Interviews and offers. This is the long, uncertain middle of the UK route and the section that most often overruns. Keep the NMC file progressing and answer any evidence request the day it arrives.

Decision point B, end of month 7. If you have no offer and your English certificate has less than a year left, decide whether to widen your search to other regions and settings, or bank the certificate for a different destination. Do not let it drift.

Months 7 to 9. Offer accepted. The employer applies for and assigns a certificate of sponsorship. Now order police clearances from your home country and any country where you lived long enough to need one, and book the immigration medical if required.

Months 9 to 11. Lodge the Health and Care Worker visa application with UK Visas and Immigration (UKVI), pay the published application fee and submit biometrics. UKVI publishes a separate immigration health surcharge for most visa routes, and it also publishes which health and care applicants are exempt from it, so check that exemption on gov.uk against your own route before you budget. Confirm the whole fee schedule on gov.uk on the day you pay, because published fees are revised.

Months 11 to 12. Travel, start work, and sit the objective structured clinical examination in the UK within the window your employer and the NMC agree. Full registration follows the pass. The UK sequence puts the employer before the final exam and before the visa, which is the structural fact to plan around.

The Australia sequence, month by month

Australia inverts two steps, because a points-tested visa needs no employer.

Months 1 to 2. Sit the English test at the level Ahpra publishes for registration. The regulator's bar is commonly at least as high as the level the Department of Home Affairs publishes for a skilled visa, so clearing the regulator often covers both, and you should still read the two requirements side by side on ahpra.gov.au and immi.homeaffairs.gov.au before you book a test date. Begin gathering the evidence ANMAC needs for a skills assessment.

Months 2 to 5. Apply to Ahpra and enter the assessment stream it assigns you. Lodge the ANMAC skills assessment in parallel. The two run concurrently and are separate transactions with separate fees.

Decision point A, end of month 5. Ahpra tells you whether your path is an outcome-based assessment or a bridging program studied in Australia. A bridging program usually implies a period of study in Australia, which is a different visa and a different budget. If that is your outcome, stop and re-plan the route before spending more.

Months 5 to 7. With a positive skills assessment, submit an expression of interest through SkillSelect for the subclass your points support, or begin an employer-sponsored application if you have an employer. Points are set by the Department of Home Affairs and turn on age, English, experience and qualifications, so calculate them on the department's own page.

Decision point B, end of month 7. If your points are below the level realistically being invited in your occupation, decide whether to improve English, add experience, or pursue state or territory nomination. Each state and territory publishes its own nomination criteria, and those criteria change during the programme year, so read the current page for the state you have in mind.

Months 7 to 10. Invitation received, if it comes. Lodge inside the short window it allows. Order police certificates and book the medical now.

Months 10 to 12. Visa decision, travel, and completion of the in-country component of the Ahpra assessment. The job search can begin before departure and continue after arrival, because no employer sponsorship is required on a points-tested visa.

The Australian sequence front-loads more of your own money, because no employer subsidises exam and travel costs. It also removes the employer as a bottleneck, which is why many nurses accept the higher upfront spend.

What the wrong order actually costs

Four common orderings and their consequences:

  • English test sat last. A nurse pays for verification, opens a regulator file and applies to employers, then discovers the writing sub-score is half a band under the regulator's floor. Everything waits on a retake, and fees already paid have their own clocks running.
  • Police clearance ordered early. Certificates obtained at the start "to get them out of the way" expire during the employer search and are re-ordered at full cost, sometimes from a country with a long queue.
  • Skills assessment obtained before English. The ANMAC outcome sits on the shelf while the applicant retakes an English test, burning part of its fixed validity period.
  • Visa lodged with a nearly expired English certificate. The certificate is inside the window on the day of lodgement and outside it when a caseworker asks a follow-up question. Authorities differ on which date they assess against, so leave margin.

Each of these costs money. A retaken English test, re-ordered police certificates, a repeated medical and a lapsed skills assessment are each sums the test providers, police authorities, panel physicians and ANMAC publish on their own sites. The expensive part is the delay: a nurse who reaches a ward in month twenty-four instead of month twelve loses a year of the salary gap, pension contributions and progression.

When this sequence does not work

The order above assumes a straightforward case. It breaks down in several situations, each of which needs its own plan:

  • You already have a firm, verified job offer from a licensed UK sponsor. The employer's timeline leads, and your sequence bends to the certificate of sponsorship date.
  • Ahpra places you in a bridging program. The route becomes a study route with a different visa, different funds requirements and a longer horizon.
  • Your qualification cannot be verified. A closed institution or lost records turns verification into the critical path, and the plan should start there instead of with the English test.
  • You have lived in four or more countries. Police clearances become the long pole and may need starting far earlier than "last" implies.
  • You are moving with a partner and children. Dependant applications, their documents and school enrolment deadlines add constraints that reorder some steps.
  • Your home council will not issue a certificate of good standing while you have an active employment bond. Resolve that before anything else.

Common mistakes

  1. Sitting the English test at the visa level and assuming it clears the register. The regulator's bar is normally higher and has sub-skill floors.
  2. Treating the two English windows, the regulator's and the immigration department's, as the same window.
  3. Ordering police certificates and medicals early "to be prepared".
  4. Paying an agent an upfront fee for a job offer. A legitimate employer or licensed recruiter does not charge you to be hired.
  5. Pausing a regulator application for months without checking whether verification or an exam pass has a validity period that keeps running.
  6. Building the plan around a fee you read once. Fees published by the NMC, Ahpra, ANMAC, UKVI and the Department of Home Affairs are revised, so reconfirm on the day you pay.
  7. Running both routes at once on a limited budget, then abandoning both when neither reaches a decision point.

Agents, recruiters and how to verify them

Sequencing is where unregistered agents do the most damage, because a person who does not understand the dependency chain will happily sell you the wrong step first. Concrete warnings:

  • Never pay an upfront fee for a job offer. Employer-paid recruitment is the norm in structured NHS and large private-provider pipelines.
  • Never pay into a personal bank account. A legitimate firm invoices from a company account and issues a receipt.
  • Treat a guaranteed visa as a lie. No agent controls a UKVI or Home Affairs decision.
  • Verify the sponsor, not the agent's word. For the UK, check the employer against the GOV.UK register of licensed sponsors, and check an adviser's status with the UK statutory regulator of immigration advisers, which publishes a searchable register of regulated advisers (the body formerly known as the Office of the Immigration Services Commissioner; confirm its current name and register on gov.uk). For Australia, check whether your adviser is a registered migration agent with the Office of the Migration Agents Registration Authority, or a legal practitioner.
  • Ask for terms in writing when an employer offers to fund exams and travel: what is paid, what is advanced, what is recovered from salary, and what happens if you leave early or fail an exam.

Frequently Asked Questions

Should I take the English test before or after applying to the regulator?

Before. Both the NMC and Ahpra need an English result to progress a registration file, and the test is the cheapest way to discover whether you meet the higher of the two bars you face. Sitting it first prevents you from paying regulator fees that sit idle while you retake a component. Watch the certificate's own clock, so avoid sitting it years before you can realistically use it.

How long is an English test result valid for nurse registration?

Health regulators commonly accept a result for a period counted from the test date, often around two years, and immigration departments may apply a different window to the same certificate. The exact rule is set by each authority, and both the NMC and Ahpra publish their own. Ask the regulator, check the subclass requirements on the Department of Home Affairs website, and have a registered migration agent confirm before you lodge anything.

When should I order my police clearance certificate?

When a visa application is imminent, with one exception: if any country you lived in is known to be slow at issuing certificates, start that one early and accept you may need to repeat it. Police certificates typically have short validity windows, so ordering the whole set at the beginning of a twelve-month plan usually means paying for them twice.

Do I need a job offer before starting registration?

For Australia on a points-tested visa, no. Registration with Ahpra and the ANMAC skills assessment are both independent of employment. For the UK, registration can start without an offer, but the visa cannot, so the employer search should run in parallel with the NMC file instead of after it. Employer-sponsored Australian subclasses obviously need an employer, and their sequence looks more like the UK one.

Is it worth running the UK and Australia routes at the same time?

Only if you can genuinely fund both to a decision point. Each register charges its own fees, neither gives credit for the other's exams, and each adds expiry dates you have to defend. Work in this order, and write dates against each line.

  1. Confirm the current English requirement on the regulator's own page for your chosen country, including every sub-skill minimum.
  2. Book the English test for the earliest date you can prepare properly for, and note the result's expiry date the day you receive it.
  3. Ask your university and home nursing council what they need to respond to a verification request, because that answer sets your true start date.
  4. Draw the twelve-month calendar on one page with every expiry date on it, including the two short windows: the certificate of sponsorship and the skilled visa invitation.
